If you are attending Prime For Life you will need to bring a copy of your ten (10)year driving record to your initial session with a HCBH clinician. You can visit the WYDOT office in person (the Afton office is open Thursdays and Fridays) to get a copy or you can mail the WYDOT form below to them with your $5.00 fee (requires 7-10 business days). The form has a place for you to indicate to fax or mail it directly to the appropriate HCBH office.
